Are older tattoos easier to remove? It’s one of the most repeated claims in the field, and people who’ve carried the same ink for twenty or thirty years often arrive at a consultation expecting a shorter treatment plan because of it.
The claim is partly true. But which part? The KirbyDesai scale – the main published tool for estimating how many sessions a tattoo will take – doesn’t score tattoo age at all.

Age matters only through what it changes. A tattoo that has genuinely faded presents less pigment, and that is captured above as amount of ink.

The scale that gets mis-quoted
The Kirby-Desai scale, published in the Journal of Clinical and Aesthetic Dermatology in 2009, is the standard method for estimating how many laser sessions a tattoo will need. StatPearls, the clinical reference practitioners use, points to it as the available means of putting a number on that question. It assigns points across a set of parameters, adds them up, and treats the total as a rough session count.
It scores six things: skin type, location on the body, ink color, amount of ink, scarring or tissue change, and layering.
Tattoo age isn’t one of them.
That hasn’t stopped it from appearing. Clinic pages and laser-industry explainers regularly describe the scale as including tattoo age, sometimes listing it among six named factors while naming six factors that aren’t quite the paper’s six. The original study is open access and the parameter list sits in its abstract, so this is checkable in about a minute.

It may run high. A 2024 retrospective evaluation found patients needed about half the sessions the scale predicted. That study covered eleven people, so it points at a direction without settling it.

The ink doesn’t fade away
The reasoning behind the age claim is that ink breaks down over the years, so an old tattoo has less pigment left to clear. The first half holds up in a limited way. The second half doesn’t follow from it.
Kirby and his co-authors address the point directly in their discussion. Citing biopsy work spanning intervals from two to three months out to forty years, they describe ink granules still present in dermal fibroblasts, sitting in a perivascular position beneath a layer of fibrosis. Four decades on, the pigment is still in the skin.
What changes is how it reads. Ink disperses, edges soften, and the epidermis above it turns over, so an old tattoo looks duller and blurrier than a fresh one. Less visible isn’t the same as less present.
There’s a further complication. As pigment becomes encapsulated in fibroblasts it can settle deeper in the dermis over time. For the older removal methods that worked by abrading skin away, that made aged tattoos harder to treat.
What age is standing in for
One of the six scored parameters does shift with age, and it’s where the claim gets its footing.
Amount of ink is scored from one to four. A tattoo that has genuinely lightened across decades presents less pigment to a laser than the same design would have at six months. That effect is real, and the scale captures it as ink density, not as age. Two tattoos of equal density score the same whether one is thirty years old and the other is new.
So age isn’t doing the work. Fading is, and fading is what a provider can see and assess in front of them. If you want a sense of how those factors combine into a number, the way session counts are estimated covers the arithmetic.
When an older tattoo is harder
Layering carries two points on the scale, and it works against age.
A cover-up places new work over existing work. Because tattoo ink is translucent, hiding an old design requires darker and denser pigment than the piece underneath, usually across a larger area. The result is more ink, arranged in more layers.
Older tattoos are the ones most likely to have been covered. Someone who regretted a tattoo in 2004 had cover-up work as the main practical option, and that history sits invisibly beneath whatever is on the skin now. It adds sessions. The trade-offs involved in fading for a cover-up run in the same direction.
The amateur confound
A good deal of what looks like an age effect is a tattoo-type effect.
Amateur tattoos are placed less evenly, sit more superficially in the dermis, and carry less pigment than professional work. StatPearls notes that this makes them easier to remove. The Kirby-Desai paper puts the average amateur tattoo at roughly 16 square centimeters against 66 for professional work, citing earlier measurements by Alster.
Older tattoos skew amateur, because studio work has grown as a share of all tattooing over the past few decades. A clinic that sees a lot of very old ink clear quickly may be watching amateur tattoos clear quickly. The two categories overlap enough to be easily confused, and only one of them is scored.
What this means at a consultation
A provider assessing an old tattoo should be working from what’s in front of them: how dense the ink is now, which colors are present, where it sits on the body, whether there’s scarring, and whether anything was placed over anything else. Age is useful to them as a clue about those things and not as a factor in its own right.
Worth asking directly what they scored and on what basis. A provider who predicts a quick job from age alone, without examining density, is working from a rule of thumb.
For scale, the original study found an average of ten sessions across its hundred patients, with a range from three to twenty. A small 2024 retrospective evaluation in the Journal of Cosmetic Dermatology found that actual treatment numbers came in well below what the scale predicted, which suggests it may run high with current equipment. That study covered eleven patients, so it’s a signal rather than a correction.
Either way, the number depends on what the ink is doing now. How the body clears fragmented ink between sessions is the other half of the picture.
